- Original Transcript of Records/ Original Scholastic Records bearing School Seal and Signature of the Registrar
- Transfer Credentials
- Certificate of Good Moral Character from Authorized Official of former school
- Student’s letter of intent
- Parent’s/ Guardian’s letter seeking admission
- Certificate of Eligibility for Dentistry (for Dentistry applicants only)
3. Pay the first payment indicated in your chosen Payment Plan using your chosen Payment Channel
4. Upload the following final requirements through this link;
