  • Undergraduate Scholarships – DOST-SEI

The S&T Undergraduate Scholarships Program aims to stimulate and entice talented Filipino youths to pursue lifetime productive careers in science and technology and ensure a steady, adequate supply of qualified S&T human resources which can steer the country towards national progress.

The Junior Level Science Scholarship (JLSS) Program aims to: provide scholarships that will finance the education of talented and deserving students in their third year of college and who are pursuing degree programs in the areas of science and technology; and ensure a steady, adequate supply of qualified S&T human resources who can steer the country towards national progress.

  • Scholarship for Dependents OWWA

A scholarship grant to qualified dependents of OFWs consisting of a maximum of Php 60,000.00 per school year until the completion of baccalaureate course (4-5 years). For incoming freshmen, the selection of scholars is determined through the DOST national examinations. Only the top 400 passers are considered for entry as freshmen; 600 slots are for those who are already enrolled in college.

Western Mindanao State University Admission | Required Documents | Application Form

STEP 1. Pay the necessary Fee for the Physical Medical Examination at the WMSU Cashier’s Office. Ground Floor Administration Building, WMSU.

STEP 2. Proceed to the University Health Center, WMSU Main Campus and inquire  about necessary requirements for Physical/Medical Examination and have your PME. When done secure PME certification.

STEP 3. Go to the College of your choice for the interview (if needed, and/or  enrolment on the appointed date. If you get disqualified in the first course you chose, you may proceed to the next college of your  choice, provided however, you meet all requirements of your next chosen college.

STEP 4. Go to the college where you qualify and enroll.

I. NEW STUDENTS (FRESHMEN – GRADUATE & UNDERGRADUATE PROGRAMS)

  1. Proceed to the College for Admission
  2. Present the following document in two (2) long brown envelope, Write Name at upper left corner)
    1. NSO Certified Birth Certificate (Original &2 photocopy)
    2. NSO Certified Birth Certificate (Original &2 photocopy)
    3. College Entrance Test Result (Original &1 photocopy)
    4. Form 138 (H.S. Card) (Original &1 photocopy)
    5. Medical Certificate (Original &1 photocopy)
    6. Certificate of Good Moral Character (Original &1photocopy)
    7. College Admission Evaluation Form/College Admission interview form (Original &1photocopy)
    8. Parent’s/Guardian Income Tax Return or Certificate of Indigency
  3. Fill up the Admission form legibly and submit to Admission officer.
  4. Photocopy the Approved Admission form (2copies)
  5. Present the original copy of the admission form to the Program Adviser for issuance of Enrolment Advising Form
  6. Fill up the Enrolment Advising form, write legibly Name, Course major (if any), College, Year Level, Semester and School Year; Subject ID, Subject Code, Subject Description, Units, Time, Days and Room.
  7. Present duly accomplished advising form to the adviser for approval.
  8. Proceed to the Encoding/Advising Center and present the ApprovedAdvising form and the photocopy of the admission form.
  9. Then proceed to Assessment forassessment of fees then pay at the cashier.
  10. Proceed to enlistment center forEnlistment to class (for confirmation of enrolment) and will be issued a Certificate of Registration (COR)
  11. Proceed to the college and present COR-Green copy.
  12. Enrollment is Complete.

II. OLD STUDENTS

  1. Secure clearance for enrolment from the College.
  2. Present duly accomplished clearance form to the class/level/program adviser for issuance of enrolment advising form.
  3. Fill up the enrolment advising form, write legibly Name, Course, major (if any), College, Year Level, Semester, School Year, Subject ID, Subject Code, Subject Description, Units, Time, Days, Room and Student Number.
  4. Present duly accomplished Advising form to the Adviser for approval.
  5. Proceed to the encoding/Advising Center and present the approved advising form and approved admission form.
  6. Proceed to Assessment for assessment of fees, and then Pay at the Cashier.
  7. Proceed to Enlistment Center for Enlistment to Class; confirmation of Enrolment. Certification of Registration (COR) will then be issued.
  8. If section/class is full an alternative class will be provided or a request for increase in class size form will be issued for approval.
  9. Return to the College and Present the green copy of the COR, to the class/program adviser.
  10. Enrolment is Complete.

III. RETURNING STUDENTS

  1. Secure Clearance for enrolment, present to the Dean your Leave of Absence Form (LOA) duly approved by the Dean of Admission for re-admission.
  2. Present accomplished clearance form and LOA to your program adviser for issuance of enrolment advising form.Present secure/ present grade from previous subject enrolled.
  3. FOLLOW PROCEDURE 3 TO 10 OF PART II.
  4. Enrollment is complete.

IV. TRANSFEREE

  1. Proceed to the University Dean ofAdmission Office.
  2. Present the following document in two (2) long brown envelopes:
    1. NSO Certified Birth Certificate (Original &2 photocopy)
    2. NSO Certified Birth Certificate (Original &1photocopy)
    3. College Entrance Test Result (Original &1photocopy)
    4. Medical Certificate (Original & 1 photocopy)
    5. Transcript of Records (Original & 2 photocopy)
    6. Certificate of Eligibility to Transfer
    7. College Admission Evaluation Form/College Admission interview form (Original &1photocopy)
  3. FOLLOW PROCEDURE 3 TO 11 OF PART I.
  4. Enrollment is complete.

V. CROSS ENROLLEE

  1. Proceed to the University Dean of Admission Office
  2. Present the following document in two (2) long brown envelope
    1. Certificate of Eligibility toCROSS-ENROLL
    2. Medical Certificate (Original & 1 photocopy)
  3. Proceed to the college where the subject/course you intend to cross enroll is offered with a duly accomplished admission application form; enrollment advising form will be issued.
  4. FOLLOW PROCEDURE 3 TO 11 OF PART I.
  5. Enrollment is complete.
