- Certified True Copy of Form 138 – no grade lower than 85 in all subjects during the third quarter of the present school year (no grades below 80 in all subjects in first and second quarters/grading periods)
- Certificate of Good Moral Character from the Principal/Guidance Counselor (with school seal)
- NSO Copy of Birth Certificate
- Barangay Captain’s Certificate of Residence
- Two copies of 1×1 recent colored picture

- Certified True Copy Report Card – with no failing grade in any subject
- Certification from the Principal that the student is graduating from Senior High School.

ADMISSION REQUIREMENTS FOR UNDERGRADUATE (FOREIGN STUDENT)
- A formal letter of application addressed to the University President seeking admission to PNU Undergraduate Program to be reviewed by the Dean, College of Teacher Development/Associate Dean
- Authenticated High School Card and authenticated Certificate of Graduation in High School/Diploma with English translation duly certified by the University.
- A computed Grade Point Average (GPA) equivalent to at least 80% or its equivalent in the PNU Grading System Students with TOEFL, IELTS, TOEIC should have a band of 6
- Students without TOEFL, IELTS, TOEIC are required to take ELP (English Language Proficiency) prior to taking the PNUAT Authenticated Birth Certificate
- Certificate of Good Moral Character
- Two copies of 2×2 latest colored picture
- A written Personal History Statement with photocopy of Passport (original Passport should be presented)
- Entrance Examination Fee of US$ 50.00 (Subject to change upon the approval of the Board of Regents)
