For Local and National Students:

1. Application fee (Non-refundable)
2. Original Form 138 (HS Card)
3. Two (2) letters of recommendation from:
   • School principal
   • District pastor/parish priest (for non-SDA applicants)
4. Certificate of Moral Character from the school last attended
5. Original NSO live birth and three (3) photocopies
6. Three photocopied marriage certificates issued by PSA (for married students)
7. Recent ID Pictures:
   • Eight (8) copies of 2×2
   • Three (3) copies of 1×1
8. For Transferees:
   • Honorable Dismissal from the school last attended
   • Certification of grades/Transcript of records
   • Three photocopied marriage certificates issued by PSA (for married students)

Admission For Foreign Students:

1. Academic record with an average grade of C+ (85%) or its equivalent from the previous school last attended (refer it to an academic committee)
2. Certificate of good moral character from the school last attended
3. Test of English as a Foreign Language (TOEFL) certification from the country of origin (refer to the academic office) for applicants from non-English speaking countries
4. A non-refundable admission fee of $1,000 is payable for two semesters
5. Paycheck for financial guarantee and airfare deposit in USD before arrival (to be specified by the Business office)

• Scholarship for Dependents OWWA

A scholarship grant to qualified dependents of OFWs consisting of a maximum of Php 60,000.00 per school year until the completion of baccalaureate course (4-5 years). For incoming freshmen, the selection of scholars is determined through the DOST national examinations.
